Program: DMV - DISABLED PERSON PARKING PLACARDS AND LICENSE PLATES

Descriptions
Provides parking placards or plates to individuals with an impaired mobility condition. Condition must be certified by a:
- licensed physician
- surgeon
- physicians assistant
- nurse practitioner
- certified nurse midwife
- licensed chiroprator
- licensed optometrist
- licensed podiatrist
Note: No medical certification need, if condition is loss of a foot or both hands (but must apply in person).
Eligibility
Must have:
lost the use of one or more lower extremities, or both hands.
a diagnosed disease that substantially impairs or interferes with mobility.
been unable to move without the aid of an assistive device.
specific, documented visual problems, including lower-vision or partial-sightedness.
Application Process
Visit the website, mail in paper application, or visit during program hours. Appointment recommended. For change of address, download a change of address form at dmv.ca.gov/addresschange or call.
Documents
Have a copy of one of the following acceptable proofs of true full name and date of birth:
- A state-issued driver's license (DL) or identification (ID) card
- United States (U.S.) birth certificate
- Valid and unexpired U.S. passport or U.S. passport card
